We are thrilled to announce the arrival of Dr. Adam Scott, joining our team in the Summer of 2024.

With a commitment to exceptional patient care and a wealth of experience in general pediatrics, with a special interest in nephrology, Dr. Scott brings a fresh perspective and expertise to our practice. We're excited to have such a talented and compassionate physician join our ranks.

As we prepare for Dr. Scott's arrival, we're working diligently to ensure a seamless transition and to provide our patients with the highest level of care. We are in the process of credentialing Dr. Scott within our office. Typically, insurance companies may take anywhere from 2 to 6 months to complete the full credentialing process. Rest assured, we are committed to minimizing any gaps in patient care by promptly scheduling appointments once Dr. Scott's credentialing is approved by the insurance companies.

FOR CURRENT PATIENTS OF DR. SCOTT'S WISHING TO TRANSFER TO PARTNERS IN PEDIATRICS:

Please fill out the transfer request below with your child's name and the most convenient contact information for us to reach you once we've obtained their medical records. By completing this survey, you are also authorizing our office to obtain the necessary medical records for transfer. Completing this form will not create any interruptions at the previous office should you require care. Please continue to see the providers at Professional Pediatrics until your records have been transferred.

Upon establishing a start date, we will promptly send a medical record release to Professional Pediatrics to request the necessary records. Subsequently, we will schedule all new patient establishment visits upon receiving these medical records, prioritizing those who may need check ups and immunizations the soonest. We kindly ask for your patience during this process. 

*Please be aware that insurance companies may classify new patient visits as check-ups, even if you've had a previous visit within the last year. This classification can impact billing and coverage, so we advise checking with your insurance provider for clarification on their policies regarding new patient visits. We're committed to assisting you with any insurance-related inquiries and ensuring transparency throughout your healthcare journey. FOR ALL OTHER NEW PATIENT REQUESTS:

We kindly request your patience as we facilitate the transfer of Dr. Scott's current patients from his previous office to ours. Ensuring a smooth transition for existing patients is paramount to us, and we want to guarantee continuity of care for each individual.
